Breast implant problems

Breast implants are generally safe, but like any medical device, they can present issues over time and are not designed to last a lifetime. It’s likely they’ll need to be replaced eventually. Many women may require further surgery around the 10-year mark, either due to problems with the implants themselves or because natural changes in the breast tissue have affected their appearance. Some common issues that may arise with breast implants include:

  • Capsular contracture
    This occurs when the scar tissue (capsule) around the implant tightens, causing the breast to feel firm, appear distorted, and may be painful. Capsular contracture is the most frequent complication with implants and can develop months or even years after the implant surgery.
  • Implant rupture
    Implants are designed to be durable, but they can sometimes rupture due to trauma, natural aging, or manufacturing flaws. A rupture may cause the implant to deflate (in the case of saline implants) or develop a leak (in silicone implants). If a silicone implant ruptures, it can lead to local inflammation and changes in breast shape.
  • Implant displacement or rotation
    Sometimes, implants may shift from their original position or rotate within the breast pocket. This can result in asymmetry or discomfort and may affect the aesthetic appearance of the breasts.
  • Breast implant-associated anaplastic large cell lymphoma (BIA-ALCL)
    BIA-ALCL is a rare type of lymphoma that can develop around breast implants, particularly textured implants. Though not a breast cancer, it can form in the capsule surrounding the implant and requires medical attention.
  • Breast implant illness (BII)
    Some women report systemic symptoms like fatigue, joint pain, and brain fog, which they attribute to their breast implants. While BII is not a medically recognised diagnosis, some patients find improvement in symptoms after implant removal.
  • Double bubble deformity and rippling
    These aesthetic issues may arise if the implant moves out of place or if the skin and tissue over the implant are too thin, causing visible folds or ripples.

Symptoms that may indicate a breast implant problem

  • Pain, tenderness, or discomfort in the breast
  • Changes in breast shape, size, or firmness
  • Visible distortion, asymmetry, or rippling of the breast
  • Skin redness, swelling, or the formation of lumps around the implant area
  • Unexplained systemic symptoms such as fatigue or muscle pain (in the case of suspected Breast Implant Illness)
  • Swelling or fluid collection around the implant (a potential sign of BIA-ALCL)

Diagnosis and evaluation

To accurately diagnose breast implant problems, a comprehensive evaluation is needed. In some cases, a biopsy may be performed if there is suspicion of BIA-ALCL or other unusual findings.

Treatment options

The treatment for breast implant issues varies depending on the nature and severity of the problem:

  • Capsulectomy
    In cases of severe capsular contracture, a capsulectomy may be performed. This involves removing the hardened capsule around the implant to restore comfort and a natural appearance to the breast.
  • Implant removal or replacement
    If there is a rupture, deflation, or significant discomfort, the implant may need to be removed or replaced. Many patients choose to have new implants placed during the same procedure, while others may opt for alternative reconstruction methods.
  • Implant removal for breast implant illness
    For those experiencing symptoms of BII, complete removal of the implant and surrounding capsule (en bloc removal) is an option. Many patients report symptom relief after implant removal.
  • Treatment for BIA-ALCL
    If diagnosed with BIA-ALCL, the primary treatment is implant and capsule removal. Early detection and intervention are crucial, and in most cases, surgery is effective in treating the condition.
  • Revision surgery for aesthetic concerns
    For issues such as rippling, double bubble deformity, or asymmetry, revision surgery may help improve the breast’s appearance and symmetry.
